COVID-19 Vaccination Scheduling
Welcome to the online scheduling tool for the City of Milwaukee’s COVID-19 vaccinations.
This site helps identify those who are currently eligible for the COVID-19 vaccine for the City of Milwaukee. Eligible individuals will be allowed to schedule their initial vaccination appointment, which can generally be scheduled 1-14 days in advance based upon vaccine availability. Please note: You will be required to show proof of eligibility at your appointment.
In order to schedule your appointment, you will need to provide a phone number and email address. Follow-up appointments will be scheduled on-site after you receive your initial vaccine dose.
